This grant-funded position offers a unique platform to collaborate with diverse teams and community agencies, ensuring our patients receive the comprehensive care they deserve.
Key Responsibilities
- Guide patients through service delivery systems and agency protocols, ensuring effortless navigation.
- Collaborate with medical case managers and clinic staff to implement seamless care plans.
- Monitor and support patient progress in HIV care, following up on referrals to guarantee appointments are met.
- Coordinate essential services such as transportation and translation to eliminate care barriers.
- Conduct detailed assessments of patients' medical and psychosocial status to tailor services effectively.
- Develop strategic care plans to assist referred families and patients in achieving care goals.
- Provide crucial referrals and linkage to medical, psychosocial, and essential life services.
- Adapt methodologies to align with specific care needs respecting cultural, physical, and developmental considerations.
- Ensure prompt documentation of services in the EHRS client record within 72 hours.
- Facilitate access to legacy programs addressing psychosocial needs within the wider community.
- Advocate for clients to bridge service gaps and dismantle access barriers.
- Engage with Legacy personnel and providers for comprehensive system navigation support.
- Collaborate with community-based institutions to foster connectivity and social support linkage.
Qualifications
- High school diploma or equivalent education.
- Minimum of 1-year paid work experience with People Living With HIV (PLWH).
- Knowledge and familiarity with Ryan White grants and services are strongly preferred.
- Experience with EPIC systems is an advantage.
- Spanish fluency or bilingual capabilities are preferred, enhancing patient communication.
